SINGAPORE: The rate of elderly patients returning to hospitals and polyclinics for additional care has dropped from 3.5 times to 1.3 times in just a year.
Another hospital is expected to join the Community Health Mapping Programme soon.
Patrick Tay, grassroots adviser at Nee Soon East Grassroots Organisations, said: “We have already agreed that we should continue our partnership even with the new (Yishun Community Hospital). I think we have rolled out quite a few programmes with the hospitals and various agencies, including preventive care as well as to minimise re-admission rate of patients from the hospitals who are our residents.”
The programme started a year ago and has catered to almost 50 per cent of elderly residents in the area who would otherwise have to go to hospitals and polyclinics for checkups.
The Barrier-Free Access Map, which was launched on Monday, shows the locations of ramps, making it easier for wheelchair-bound residents and their caregivers to move around Yishun.
